ID for someone age 16 and under: 'Proof's Needed'
....Original birth certificate or certified copy (**photostat copies NOT ACCEPTED**)
....Proof of address: (Envelope or magazine or bill delivered by post office within 30 days of request 'ID' is made) Proof must have person's Name and Current Street Address.
....SOCIAL SECURITY CARD, or W-2 TAX FORM, or PAY STUB WITH SOCIAL SECURITY NUMBER.
....One of the following types of 'ID' are also required: Social Security Card, Passport, Baptismal Certificate, Benefits Card, Insurance Card, recent yearbook picture w/name near or under picture, picture w/name under it from newspaper, current school 'ID' card w/name imprinted on it.
....Parent or Guardian must be present and they must have a Photo 'ID" with them. (A Guardian must show legal guardianship papers.)
